As a Hospice LPN you will provide skilled and compassionate nursing care to patients and families in their home or community under the supervision of a Registered Nurse and direction of the Plan of Care.
You will be responsible for visiting patients to assist in their end of life care, the documentation of care given, and chart maintenance to meet both regulatory and billing requirements.
Provides services in accordance with organizational policies and procedures and regulations that define scope of practice Prepares clinical and progress notes on the relevant and specialized nursing services he/she provides Assists the RN and/or physician in performing specialized duties related to end-of-life care Assists the RN in carrying out the plan of care Prepares equipment and materials for treatment adhering to aseptic technique as required Assists the patient in learning appropriate self-care techniques Communicates changes in patient condition and needed changes to the plan of care to appropriate team members and caregivers Participates in staff and IDT meetings as requested Ensures the maintenance and confidentiality of client records and employee information in accordance with Confidentiality policy Attends nursing home case conferences as requested Assists with record reviews if requested Contributes to the effectiveness of case management and coordination of services through open communication Follows directives of case manager and supports the efforts of case coordination Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company #NURSE Recommended Skills Billing Case Management Certified Nurse Practitioner Clinical Works Confidentiality Hospice Estimated Salary: $20 to $28 per hour based on qualifications.
